For one, that's not the policy. Just go read it and find where it says it is always the employee's fault. Secondly, I have had a couple of injuries and accidents and none of them were my fault and none of them were counted as preventable or avoidable. Lastly, I've been on my station's SCIT team and the team at my previous station. We reviewed all accidents and injuries and we, as hourly employees, often deemed an accident or injury as preventable or avoidable because that's what it was. We also had situations where the manager and sr. mgr said that it was not avoidable.</p><p></p><p>Obviously I cannot dispute what you heard so I'll gladly take what you said at face value and say that that management team is a sorry sack of @#$%. Fortunately there are ways to have these things reviewed so that someone outside that team can see what is going on.</p></blockquote><p></p>
