UPS Pilot Fatigue in Focus as NTSB Weighs Cause of Crash - Bloomberg

Two fatigued United Parcel Service Inc. (UPS) pilots committed multiple errors before their cargo plane slammed into a hillside last year in Alabama, according to U.S. investigators who are meeting to decide the cause of the crash.

The copilot also hadn’t taken opportunities to sleep during her time off between flights, which may have contributed to those errors, Katherine Wilson, a National Transportation Safety Board investigator said in an opening presentation at a hearing today. The safety board is still debating what caused the Aug. 14, 2013, accident and will vote on a conclusion later today.
