Not sure on the first part, 1025 hours for 12 weeks fmla.
To get 6 weeks, must be employed 36 months and have 625 hours.
Page 47 of the contract.

As a part timer, if the supervisor that does your payroll codes your time card wrong, Teamcare will be happy to disqualify your medical coverage. By the time you get it straightened out, the bill could end up in collections, depending on how nasty your care provider is... COBRA notices won't show up in your mailbox, until an entire month has gone by.

If you didn't qualify for 6 weeks of FMLA, you could easily end up paying any medical bills that weren't covered. Or, you could end up paying the ridiculously expensive COBRA payments.
