Did UPS not mail one to the address you had on file while working at UPS? You should have received it by February 1st.

yea i recieved one but it somehow lost itself, ups hr is closed right now, and i dont know my employee id or password

I'm thinking it's tax related, it sounds like your on the right path.

I would just wait until Monday and call UPS to ask for another one. It's not like the IRS is open today either. You got time bro...

You do realize the deadline for filing taxes or an extension is APRIL 15???
And you do understand this is the month MAY????
You are nearly a month overdue to file.
If you were hoping to get a refund, you may now get nothing once penalties are figured up.

He's has to use at least part of his employee ID to punch in and out everyday... So he should have some clue what it is...
