So I was only approved for a 3 month DOT card for high blood pressure. It’s under control now but my DOT is almost up. Does ups pay for the renewal or do we go ourselves?

Lmao maybe I should’ve phrased that better. When I originally got the physical they gave me a referral form to get my physical. Do I need another one? Or do I just contact the doctor myself and set an appointment
