Will I still receive health insurance coverage while on unpaid FMLA?

You're only eligible for 12 weeks of leave and you keep your medical.
If you take an extra month off after the 12 weeks, you will most likely have to pick up the COBRA policy.

Pretty much the title. I'm going to be on unpaid FMLA for about 4 months. I want to know if I will still be covered by Aetna.

How did you get the extra month approved? Or did you? That will impact the answer to your question. As @Turdferguson and @Mugarolla pointed out you are only entitled to 12 weeks, or about 3 months under FMLA.
