I dont want to be a liability and will not screw this up but I loaded full carts today, when a super walked up to me at the end of shift and had me sign a document of how many sequence numbers i had made mistakes with is this a write up or procedure?

Whats up, How do you go about claiming and filing an injury? What is the time frame to do so? How do you collect workers compensation?
