Disqualification Question

When a qualifying driver gets disqualified, is management supposed to tell you directly that you’ve been disqualified and have you sign a paper? I was qualifying in late October and they pulled me off the road in mid November because I had a slight mishap of not bringing back my air pickups back to the building in time. I did drive during peak but that evidently doesn’t count towards my 30. I just wanna know if I actually got DQ’d. Is a DQ letter supposed to be set in place when a driver-to-be gets actually disqualified? I just want to know before 120 days goes by, so I can possibly continue the rest of my 30 and get this done.
