Box 189161 Plantation, FL 33318-9161 1-866-825-0186 Hours of Operation: Monday - Friday 8:00 a.m. to 8:00 p.m. ET</strong></p><p> </p><p> Once your STD claim is approved (could take days, weeks) you will get paid a percentage of your weekly part-time pay. </p><p> </p><p> When you come back from the 8 weeks your doctor can give you a note for your supervisor saying you are fit for duty.
