You said the doctors had to report what their patients say, which is not necessarily true. It's in the link. There is a list put out by the manufacturers of known side effects, and one of possible side effects. If a patient is suffering from some complication that's listed on the one list, Doctors are required to report it. On the other list, they are recommended to report it. Some symptoms the doctors can verify that the patient is suffering, some they can't, but it isn't as simple as "they are required to report whatever the patient says".

Likely, but it's unlikely that it's always possible to prove causal connections even when they exist. This is why double blind random controlled experiments are necessary.
