How do they figure 60% of your base pay???

Im proud to say I've never had a std. I feel sorry that you've contacted one. You should always use protection and stay away from people who are very sexually active with multiple partners. Good luck

Back on topic our STD is 50% pay; TO A MAXIMUM OF A WHOOPING $170.

For me, they wanted the previous 3 months, not including the current month I went out. It is easiest to go to UPSers.com and print off the pay stubs that the provider will need.
Call them and ask, they will be your best source of information.
Edit: mine is based on worker's comp, so your case may be different. No matter, call your insurance provider, they will know.
