How to print my DOT medical forms

You print the forms ON YOUR CENTER'S PRINTER ON THE CLOCK! You are an idiot if you do it at home unpaid.

Feeders used to furnish them about 2 weeks before your card expired. Then they stopped that and said just get them at the Dr.'s office. Then when I'd get there, they would ask me if I had my paperwork from work filled out yet.

Crap...I’m due for my DOT physical soon. What I dread is having to go to the DMV and waste a whole day up there waitin around to renew my license...

It helps to have an authorization form from your supervisor. I went to an authorized medical provider a few months ago without it.

Thankfully they accepted my UPS Id as proof that i was authorized to get the test but I don’t know if that is protocol for all testing centers.
