Continental mechanic held responsible

The 2000 crash of the Concorde blame was placed on mechanic and Continental Airlines. The strip of metal he attached on the DC-10 fell off, the Concorde ran over it, blew a tire, fuel tank damaged and then crashed with over 100 people on board. The article fails to show why Continental did not take all the blame for the mechanic installing the strip per their maintenace program. I wonder if UPS would accept all blame if one of us, using their manuals, tooling, and parts were to end up in the same situation.
