Your salary on 12/1/14 is what is used for the calculation.
So if the factor is 70% & you make $5000/month with $10,000 in UPS Stock (not counting 401k holdings):
Performance Incentive Award = Annualized salary as of 12/1/14 ($60,000) x Your Performance Incentive Target (17% for a supervisor) x MIP factor (70%) = $7,140
Ownership Incentive Award = Value of Your UPS Holdings as of 12/31/14 ($10,000) x Your Ownership Incentive Target (3.50% for a supervisor supervisor) = $350
MIP Award = $7140 + $350 = $7490
Half will be awarded via your selection (cash, 401k, stock) & deposit on Fri 3/6 (minus taxes). The other half will vest 20% at a time over a five year period.
