I think it varies by area, however when you don't work for a whole week, UPS doesn't pay into your benefits. What should occur is you get a letter sent to you by the union notifying you that your benefits will lapse unless you pay the portion that UPS is no longer covering. If you pay it, you're fine, if you don't, they lapse until your return.

Also, for the people commenting to "Hire a lawyer".....there's nothing a lawyer can do unless UPS/Claims adjuster is denying you treatment. I'd recommend contacting your steward who will reach out to labor and hopefully get the issue rectified.
